Smoking Cessation Forms 

From 1st July 2014, minimum data set (MDS) information will be validated and submitted to the national smoking cessation database by the Pharmacy Care Record (PCR) application. More information on the use of PCR to support smoking cessation can be found by clicking here and scrolling to the bottom of screen when the webpage opens up.

Local Claims

Claims for local payments should be submitted via the e-claim form sent out by email from National Services Scotland (NSS) on a monthly basis and returned to the e-claim mailbox NSS.psd-cpclaims-ero@nhs.net.
