There's no place like home.

It is also best for our patients' health that they leave hospital as soon as they are well enough to do so.

Hospital to Home is our community based service that aims to support people with their recovery in their own homes after hospital discharge. This involves partnership with colleagues in our hospitals, social workers, community teams and care providers with an emphasis on working with the individuals receiving care to regain as much independence as possible.

So far, Hospital to Home has been able to accommodate over 500 service users, with some very positive feedback received.

Recent feedback from Care Opinion states: “Use of the new Hospital to Home service is absolutely fantastic. Allowed my mum to leave hospital and has meant that she can stay in her own home as she does not want to be re-admitted to hospital any more. All staff have been extremely caring and respectful - always happy to go the extra mile.” 

Rob McCulloch-Graham, Chief Officer Health & Social Care, said: “This re-ablement approach looks to maximise the early rehabilitation potential of people during the early weeks of care. Through our multi-agency approach to this service, we are able to help develop people’s confidence and skills so that they can carry out activities themselves, enabling them to continue living at home.”

Erica Reid, Associate Director of Nursing and Allied Health Professionals/Chief Nurse Health and Social Care Partnership, added: “People cared for by this service are regaining independence, their care needs are reduced and subsequent admissions are also less. I am delighted that people receiving this service have been providing very positive feedback of their experience.”
