A simple nurse led tool used to support and encourage early catheter removal.
The catheter must be removed as soon as clinically possible following insertion. Any delay in removal creates a significant risk serious of infection.
- Haematuria Visible?
- Obstruction – urinary?
- Urology surgery?
- Decubitus Ulcer – open sacral or perineal wound in an incontinent patient?
- Input/Output fluid monitoring?
- Not for resus/comfort care/physician required?
- Immobility due to physical constraints e.g. unstable fracture?
If NO; then remove the catheter - Medical approval is not required unless concern exists.
How is this used?
EVERY in-patient must have a Nurse Led Catheter Removal data sheet, (community patients at the discretion of the district nursing team). This sheet must be completed every day if all answers are NO the catheter must be removed.
Is medical approval required?
For all routinely catheterised patients medical approval is NOT required prior to removal by the nurse, subject to following this policy and indication for removal by assessment.
Where a medical or nursing concern exists discussion should be held with the multidisciplinary team prior to catheter removal
