If you an older person living in a care home or currently in a community hospital and have mental health problems, delirium or dementia, you may received support from the Community Hospital and Care Home Assessment Team (CHAT). The team is made up of different health professionals including psychiatrists, nurses, psychologists, support workers and an occupational therapist. CHAT will also work closely with your care team to ensure the support provided is meeting your needs.

CHAT works as a team using the Newcastle model. You may see an Occupational Therapist within the CHAT if you are having difficulties with occupation (meaning anything we do). An Occupational Therapist will complete an assessment and work with you, your family and the care staff to develop a care plan that promotes your independence and engagement in things that are meaningful to you. They may also assess the environment to identify anything that is impacting on your wellbeing and suggest appropriate changes.
