Where We Are

Based in Kelso Health Centre with satellite offices in Jedburgh and Coldstream Health Centres.

We cover Kelso, Jedburgh, Coldstream and Greenlaw areas.

Who We Are

Our team consists of:

  • 2 Physiotherapists
  • 2 Occupational Therapists
  • 2 Staff Nurses
  • 4 Healthcare Support Workers
  • 1 Secretary
  • 1 Team Lead

Our office is manned Monday - Thursday 8:30am until 3:30pm, and every second Friday 8:30am until 1pm

What We Do - The Team Provides

  • To provide access to domiciliary occupational therapy, physiotherapy and nursing services within Cheviot locality (linked with medical practices in Kelso, Coldstream, Jedburgh and Greenlaw)
  • To support prevention of hospital admission for identified patients who require therapy services at home
  • To support safe and timely discharge from the Borders General Hospital and community hospitals
  • To support long term conditions agenda through anticipatory care
  • To support falls agenda

Who's Eligible? Referral Criteria

  • Adult resident (16+) in Cheviot locality (linked with medical practices in Kelso, Coldstream, Jedburgh and Greenlaw)
  • Client has a physical trauma or illness, disability or exacerbation of a long term condition resulting in loss of functional ability.
  • Client who requires physiotherapy and/or occupational therapy to stay at home (support prevention of admission)
  • Client who requires physiotherapy and/or occupational therapy to support timely and safe discharge from the Borders General Hospital and community hospitals
  • Client is required to be medically stable at point of discharge from hospital to access domiciliary service
  • Client to have identified physiotherapy and/or occupational therapy needs or require an anticipatory care plan to support long term conditions management in the community (nursing role)

Or telephone 01573 227939 to make a telephone referral where you will be asked questions by the secretary so that your referral can be passed to the most appropriate member of staff for your first assessment.
