About our service:

We provide early identification, interventions and support for children and young people and their families who are at potential risk of developing physical, emotional, behavioural or mental health problems.

We provide a service where children and young people can be cared for within their own home wherever possible.

We co-ordinate care by ensuring robust communication between all agencies; Integrated Children’s services, Learning Disability Team, Education, General Practitioners, Police, Child Protection, EYCAT, Social Work, CHAD, Public Health Nurses, other Health Professionals working with children locally and in Children’s Hospitals.

Reduce hospital admission and facilitate early discharge.

We care for children with complex health care needs, child protection, long-term care, continence support/referrals, palliative care & bereavement support, home ventilation and home oxygen support, enteral feeding, urinary catheterisation, central line & port-a-cath management/access, wound management, artificial airway care, oncology shared care, transitions into adult services.

Education & Training: training and education of partner agencies including Aberlour Respite, Education- including Borders College and Private Nurseries, newly qualified nursing staff, undergraduate nursing students, parents & carers. 

Facilitate Borders Forget-Me-Not Group, Bereaved parent support Group, monthly.
