Waverley

A short term rehabilitation facility (based in Galashiels) with 16 beds for people who no longer need to be in hospital but require some additional support to regain their independence before returning home.

People are referred by OT/ PT staff where it is felt that they may benefit from rehabilitation following illness or injury. While staying at Waverley, they will be seen by an occupational therapist and/or physiotherapist who will set goals with them and work with them to build confidence to promote independence and help them manage at home more efficiently.

Cheviot Community Healthcare Team (CCHT)

To provide access to domiciliary occupational therapy, physiotherapy and nursing services within Cheviot locality (linked with medical practices in Kelso, Coldstream, Jedburgh and Greenlaw). To support prevention of hospital admission for identified patients who require therapy services at home. To support safe and timely discharge from the Borders General Hospital and community hospitals.

Home First

A short term support service used to prevent hospital admission or to assist transition home from hospital. The team is set up with nursing co-ordinators for each locality area. There are Health Care Support Workers (HCSW) who supply the assistance to patients.

It can bridge the gap while awaiting the restart of a package of care; a start date of care needs to be confirmed. The support could include personal care, meal preparation or medication prompt only. They are unable to administer medication, as not nursing trained. Equally they could assist with a safe and well check. 

Within the service, there are Physiotherapists and Occupational Therapists for each locality except Kelso area, as the Cheviot Team cover this area for the rehabilitation. This allows the patient's rehabilitation to take place within their own environment. A telephone discussion must be had with the appropriate allied healthcare professional (AHP) staff member to review the referral being made. 

For an inpatient, the patient can be referred by Occupational Therapist, Physiotherapist, nursing staff or social work. In the community to prevent admission, a referral will come directly from your GP (you are not able to self refer to the service). If the referral is only for AHP referral, it must be made by an AHP.
