This leaflet is for anyone who has come into hospital for medical treatment. It is also for family, carers, and friends.
This policy describes the responsibilities of patients, relatives/carers and the Scottish Borders Health & Social Care Partnership in relation to discharging patients from hospital when they are no longer requiring hospital care.
